Pediatric Cardiac Interventions

Best pediatric cardiologist in Mumbai- Dr Supratim Sen. Senior Interventional Pediatric Cardiologist Dr Supratim Sen doing a complex cardiac intervention in the biplane cardiac cathlab

In the present era, a lot of structural interventions can be done in the cath-lab by minimally invasive techniques

Cath-lab procedures are as effective as surgery with faster recovery in these specific situations

Types of Pediatric Cardiac Interventions done by Dr Sen

  • •PDA device/ coil closure
  • •ASD Device closure
  • •VSD Device closure
  • •Balloon Pulmonary Valvotomy
  • •Balloon Aortic Valvotomy
  • •Balloon Mitral Valvotomy
  • •Balloon atrial septostomy
  • •Coarctation balloon angioplasty and stenting
  • •PA stenting
  • •RVOT stenting

Types of Neonatal Cardiac Interventions done by Dr Sen

  • •Balloon Aortic Valvotomy
  • •Balloon coarctoplasty
  • •Balloon Pulmonary Valvotomy
  • •Balloon atrial septostomy
  • •PDA stenting
  • •RVOT stenting

The PDA Stenting Program at SRCC Children’s Hospital

  • Dr Sen was instrumental in setting up the PDA stenting program at NH SRCC Children’s Hospital
  • In the last few years, we have done around 50 PDA stents in neonates with a weight of 2.5-3 kg, with excellent immediate and midterm results
  • We do most of our stenting with a hybrid carotid cutdown and flip-technique
  • PDA stenting is a life-saving intervention required in critically ill neonates with duct-dependent systemic circulation

Is there a minimum age/ weight for cath-lab interventions?

  • No for balloon angioplasty
  • Timing depends on the lesion and symptoms
  • Even a 1 kg preterm baby can undergo balloon dilation of coarctation or BPV/ BAV if indicated
  • For PDA device, special PDA device for preterm infants as small as 800 g
  • For VSD device, around 5-6 kg
  • For ASD device, around 10 kg

Transcatheter Interventions vs Open Heart Surgery

Advantages of Transcatheter Interventions

  • Quicker recovery
  • Less pain
  • Quicker discharge
  • No surgical scar
  • No cardiopulmonary bypass

Disadvantages/ Limitations of Transcatheter Interventions

  • Not all congenital heart defects can be treated with transcatheter interventions- surgery is the only option in many
  • Age and weight limitations
  • Delivery sheath size required vs patient’s vessel size
